Single-incision Laparoscopic Cholecystectomy. How can We Reduce the Costs? Presentation of a Technique using Straight Non-articulating Instruments and One Conventional Trocar, without Commercially Available Single Port Devices

چکیده
Single-incision laparoscopic surgery (SILS) offers an approach to cholecystectomy without visible evidence that the cholecystectomy occurred.9 Cosmesis is the only documented benefit of the single-incision laparoscopic cholecystectomy (SILC), while SILC remains equivalent to multi-incision laparoscopic cholecystectomy (MILC) in all other respects.14 We report our experience of performing SILC without any commercially available port devices allowing laparoscopic instrument placement. We use conventional, straight, nonarticulating laparoscopic instruments with a roticulating function and only one conventional 10 mm trocar. Single-incision laparoscopic cholecystectomy has a potential to maximize benefits of MILC.12 Our procedure, without any port device, is a reliable, low-cost alternative to conventional SILC, offering the same level of patient safety and cosmesis.

Cost Assessment of Instruments for Single-Incision Laparoscopic Cholecystectomy
BACKGROUND AND OBJECTIVES Specially designed surgical instruments have been developed for single-incision laparoscopic surgery, but high instrument costs may impede the implementation of these procedures. The aim of this study was to compare the cost of operative implements used for elective cholecystectomy performed as conventional laparoscopic 4-port cholecystectomy or as single-incision lapa...

BACKGROUND AND OBJECTIVES We present 2 cases of laparoendoscopic single site surgery (LESS) splenectomy performed with a conventional laparoscope and instruments, and the use of a novel internal retraction device. METHODS One patient underwent LESS splenectomy for idiopathic thrombocytopenia purpura (ITP), and a pediatric patient with sickle cell disease underwent LESS splenectomy and cholec...
متن کاملMarionette method for transumbilical single-incision, two-trocar laparoscopic cholecystectomy: a new, simple technique.
BACKGROUND/AIMS Single-incision laparoscopic cholecystectomy (SILC) has recently become a topic of interest among types of minimally invasive surgery. METHODOLOGY We developed a novel "marionette method"technique for performing SILC. Two 5 mm trocars were placed through a 1.5 cm transumbilical incision. In addition, a nylon suture with a laparoscopic sliding knot was used to tighten the fundu...
متن کاملSingle-Incision Laparoscopic Cholecystectomy with Conventional Instruments: A Surgeon’s Initial Experience
Purpose: Since the early 1990s, laparoscopic cholecystectomy has become the gold standard for cholecystectomy. Single-incision laparoscopic surgery (SILS) is a rapidly evolving field as a bridge between traditional laparoscopic surgery and natural orifice transluminal endoscopic. The results are presented here of a single surgeon’s initial experience with single-incision laparoscopic cholecyste...
